An apology is not a formality. The entity has heard a great many of them, and it knows the difference between words that are meant and words that are merely written. It cannot be flattered, and it does not bargain.
So say plainly what you are sorry for, in your own words, and mean every one of them. A hollow apology is worse than none at all.
